How Long Does DirectMax Take to Work? Onset, Duration and Safety

If you want to know how fast DirectMax works, start with the seller’s own numbers. DirectMeds says the rapid-dissolve tablet typically works within 15 minutes or less, and that most patients have effects lasting 24–36 hours. Those figures are the seller’s, so the medicines inside it matter too: sildenafil and tadalafil each have FDA-approved labels with their own timing and safety rules.

How Long Does DirectMax Take to Work?: Safety, Label, Side Effects

How fast does DirectMax start working, and when should you take it?

The DirectMeds DirectMax sales page says effects come “in 15 minutes or less,” and its footnote adds that “Some men may feel effects in as little as 15 minutes, while others may take longer.” The same page says DirectMeds recommends taking DirectMax “about 30 minutes before intimacy.” One of its images reads “Ready in 15 mins.” Ask your prescriber what timing fits your prescription, rather than setting a schedule yourself.

For the single ingredients, MedlinePlus gives the timing from their labels. For sildenafil tablets, the MedlinePlus sildenafil information says the best time is about 1 hour before sexual activity, that you can take it any time from 4 hours to 30 minutes before, and that a high-fat meal makes it take longer to start working. For tadalafil, the MedlinePlus tadalafil information says it is usually taken at least 30 minutes before sexual activity and not more often than once every 24 hours. Those facts describe the single-ingredient medicines, not DirectMax.

Studies of these ingredients in other forms give background. A 2018 study of 20 men with psychogenic erectile dysfunction, published in Frontiers in Pharmacology (De Toni et al.), found that a dissolving sildenafil film reached higher blood levels early on than a standard 50 mg film-coated tablet. A 2021 randomized trial of 120 men (Sangkum et al.) supported the general feasibility of dissolving sildenafil strips. Neither tested DirectMax, so the 15-minute figure remains the seller’s own claim.

How long do DirectMax effects last?

The sales page says “Most patients experience effects lasting 24–36 hours.” It adds that because DirectMax includes long-acting ingredients, duration “may vary depending on your metabolism and overall health,” and that with liver or kidney conditions, or certain medications, effects “may last longer.”

The dosing limits on two DirectMeds images disagree. One reads “Maximum 1 dose per 24 hours” and the other reads “Maximum 1 dose per 36 hours.” Ask your prescriber which limit applies to you before you take any dose. For a fuller look at the duration claim, read how long does directmax last.

What is in DirectMax, and is it FDA-approved?

DirectMeds names three ingredients: sildenafil, tadalafil and apomorphine. Its sales page says sildenafil helps you get harder faster, tadalafil helps you stay hard longer, and apomorphine boosts arousal. Strengths are sold as Low, Medium and High, and you should ask for the milligrams of each ingredient in your strength before you order.

DirectMeds calls DirectMax a “prescription-only, doctor-reviewed ED solution,” and its terms say prescriptions are filled by licensed third-party compounding pharmacies. The FDA’s FDA compounding questions and answers states: “Compounded drugs are not FDA-approved. FDA does not verify the safety, effectiveness or quality of compounded drugs before they are marketed.” DirectMax therefore falls in the compounded category, not among FDA-approved finished tablets. Sildenafil and tadalafil are FDA-approved medicines in their own products, such as Viagra and Cialis, and a compounded combination of them is a separate product.

The sales page also describes the formula as “3x more effective” and says 89% of men prefer apomorphine plus sildenafil to sildenafil alone. It footnotes that figure as “Based on a customer survey of active DirectMax patients.” A 2020 systematic review and meta-analysis of sublingual apomorphine found it could be an option for some men who cannot take PDE5 inhibitors, the drug class that includes sildenafil and tadalafil. That review covers apomorphine alone, not this combination.

Key finding

The sales page also describes the formula as “3x more effective” and says 89% of men prefer apomorphine plus sildenafil to sildenafil alone.

MedlinePlus’s tadalafil page also says the medicine does not increase sexual desire, and does not prevent pregnancy or sexually transmitted infections such as HIV.

What are the side effects and warning signs?

The sales page lists these as the most commonly reported side effects. The MedlinePlus tadalafil page lists several of the same ones.

Side effect Where it is listed
Headache and flushing Sales page; tadalafil label on MedlinePlus
Nausea Sales page; tadalafil label on MedlinePlus
Dizziness Sales page; tadalafil label lists it among signs to call your doctor about
Indigestion Sales page; tadalafil label lists indigestion or heartburn
Back or muscle pain Sales page; tadalafil label lists pain in the back and muscles
Nasal congestion Sales page
Blue tint or blurred vision Sales page; tadalafil label warns about blurred vision and a blue tinge to objects
Irritation under the tongue Sales page, which attributes it to the dissolving formula

The sales page says to seek emergency care for an erection lasting more than 4 hours, sudden vision loss, sudden hearing loss, chest pain or severe dizziness. The MedlinePlus sildenafil and tadalafil pages list the same kinds of signs, along with hives, rash, and trouble breathing or swallowing. The 2008 Cialis prescribing information on the FDA site notes that it may not be the latest label. It says you should stop and seek medical care for a sudden loss of vision in one or both eyes, and seek prompt medical attention for a sudden decrease or loss of hearing.

Warning sign What to do
An erection lasting more than 4 hours, or one that is painful Get emergency care now. The tadalafil label says call your doctor immediately or get emergency medical treatment.
Sudden vision loss Stop and get medical care now, as the 2008 Cialis label instructs.
Sudden hearing loss, sometimes with ringing or dizziness Seek prompt medical attention now; the labels say call your doctor immediately.
Chest pain or severe dizziness Get emergency care now, as the sales page says.
Fainting Call your doctor immediately, as the MedlinePlus sildenafil information says.
Hives, rash, or trouble breathing or swallowing Get emergency medical treatment, as the tadalafil label says.

Who should not use it: the sales page says not to use ED medications if you take nitrates for chest pain or riociguat, “as this can cause a dangerous drop in blood pressure.” The MedlinePlus sildenafil page says your doctor will tell you not to take it if you use street drugs containing nitrates, often called poppers. The MedlinePlus tadalafil page names riociguat (Adempas) and nitrates such as nitroglycerin. The sales page also says to tell your provider about heart problems, low blood pressure, a recent heart attack or stroke, kidney or liver disease, and eye conditions.

Because DirectMax combines two PDE5 medicines, the tadalafil label matters here too. After sudden vision loss, it says not to take more doses of tadalafil or similar medicines such as sildenafil until you talk to your doctor.

How much does DirectMax cost, and how do you get it?

DirectMeds says you complete an online medical questionnaire, and that its licensed physicians customize your dose. Its terms say availability is limited to states where offered, and that you must be in the same state as the shipping address during the consultation. The sales page says “Max performance shipped in 1-2 days.” DirectMeds’ reviews page says most patients receive their first shipment within 3 to 5 business days of completing the medical evaluation, as of October 27, 2026.

Prices as of November 2, 2026; check the checkout for today’s. The sales page FAQ lists $8.78 per dose for Low, $9.28 for Medium and $10.78 for High, in standard 6-dose packs, and says your exact total appears before checkout. A separate DirectMeds product page seen on October 20, 2026 showed “as low as $79.00” with “$119.00 every month thereafter,” so compare the two before you pay.

Can you cancel, and is there a refund?

The sales page says the product is “100% refundable” and lists a “Performance Guarantee.” The DirectMeds terms page, last updated April 16, 2026, says returns and exchanges of prescription products are accepted only for incorrect medication, damaged products, products not meeting safety standards, unshipped products, or medication denied by a Direct Meds doctor. Under those terms, medication already received is not refundable. Read the terms before you order, because the sales page and the terms say different things.

Cancellation also has two versions. The sales page says memberships “can be canceled anytime directly through your Direct Meds Patient Portal.” The terms say you can attempt cancellation within 24 hours if the provider network has not yet received the order, and that after 24 hours cancellation is generally unavailable. For subscriptions, the terms describe turning off auto-refill at least 48 hours before the next processing date. Contact support at (888) 696-7176 or help@directmeds.com, daily 9am–9pm EST, and ask which rule covers your order.

Could you get these medicines from your own prescriber?

Yes. A prescriber can decide whether a single FDA-approved medicine suits you, and you can compare that total cost, pharmacy and follow-up with any DirectMax quote. Use the directmax safety checklist to prepare questions for that visit. For a side-by-side look at DirectMax and two other ED services, see directmax vs keeps powerhouse vs medvi quad.

Before you order

  • Ask for the milligrams of sildenafil, tadalafil and apomorphine in your strength.
  • Ask which prescriber and which pharmacy will fill your order, and check that pharmacy’s state license.
  • Confirm the total first payment, the renewal charge and the cancellation deadline.
  • Ask which dosing limit applies to you, 24 hours or 36 hours.
  • Tell your prescriber about every medicine you take, including nitrates, riociguat and blood-pressure medicines.
  • Check the Direct Meds BBB profile. The seller’s reviews page claims a BBB A- rating, so see that for yourself. You can also read the Direct Meds Trustpilot reviews.
